Your First Visit

Before Your First Appointment

To lay the groundwork for your fertility care plan, and to make our conversation as productive as possible, we first need to understand more about your medical history. 

We ask that you complete your new patient forms five days in advance of your appointment and sign an electronic release form allowing us to access medical records from other healthcare providers.

What to Bring to Your First Appointment 

We ask that you arrive 15 minutes before your scheduled appointment and please bring the following items: 

  • Insurance card
  • Photo ID
  • Insurance co-payment or self-payment 

What You Will Achieve During Your First Appointment 

During our first consultation, we’ll review your medical history, discuss your goals, and talk through treatment options that seem best aligned to your situation. You’re encouraged to ask questions and share any information you feel we should know. From there, our team will reach out about next steps.

What Happens After

Together, we’ll figure out a timeline for your treatment and outline what you can expect at each step. Our team will be in communication throughout your journey, and you can reach out to us any time with questions or concerns.
